Staff Application & Reference Forms
Here is the 2010 Camp Staff Application and Reference Forms.
- The Staff Application needs to be printed, filled out by yourself. The reference form needs to be printed out 3 times; and filled out by your Pastor, Teacher and Work Reference.
Returning staff need to fill out the
Wh-4 Form (Indiana State Taxes) and the
W-4 Form (Federal Taxes) These forms HAVE to be submitted to the Conference office prior to your first day of work. This is a United States Federal regulation so please be prompt with turning these forms in. For first time camp staff, you will be required to fill out an
I-9 Form.
- The Reference Forms need to be mailed to Pastor Charlie by the person who is filling out the reference. The applicant is not to see the reference form once it has been filled out. Have the 3 individuals send it to:
